Home Health and Hospices

Home health services help adults, seniors and pediatric clients who are recovering after a hospital or facility stay or need additional support to remain safely at home and avoid unnecessary hospitalization. These Medicare-certified services may include short-term nursing, rehabilitation, therapeutic and assistive home health care.

Hospice care is designed to give supportive care to people in the final phases of a terminal illness.  The focus is to provide comfort and quality of life rather than a cure. Hospice programs can provide services at home as well as nursing homes, freestanding facilities, or within hospitals. Hospice utilizes a multidisciplinary team approach that includes physicians, nurses, social workers, aides, chaplains and volunteers in providing care.
